Intergrated Care Coordination supports older people to live independently in the community.

 

It can provide you and your family/whaanau with information about health and community services for older people and link you to the appropriate services.  It offers health information to community groups about ageing positively.

Your doctor and practice nurses work closely with the Integrated Care Coordination team and can refer you to our service.
