East Health Trust is a Primary Health Organisation (PHO) and was established on 1 July 2003. We cover covers the Howick, Botany, Pakuranga, Beachlands, Clevedon, Flat Bush, Maraetai, Kawakawa Bay and Pukekohe. Our enrolled population is approximately 122,000. About Us Welcome to East Health Trust, a Primary Health Organisation (PHO). PHOs are funded by Health NZ to support the provision of essential primary health care services, mostly through general practices, to those people who are enrolled with the PHO in the Howick, East Tamaki & Franklin areas. East Health Trusts' aim is to support services to maintain or improve the health of the enrolled PHO population. East Health Trust was established as a PHO on 1 July 2003 and covers Howick, Botany, Pakuranga, Beachlands, Clevedon, Flat Bush, Maraetai, Kawakawa Bay and Pukekohe areas. East Health represents 19 general practice clinics in the East Auckland and Franklin area. To enrol with a practice, please contact them to discuss which doctors are available to take new patients. Clinics East Health represents 19 general practice clinics across the East Auckland, Franklin and Pukekohe areas. General practices are private businesses and set their own fees for consultations and other health services. While the fees charged must be within a certain threshold agreed to by district health boards (DHBs), and PHOs, the level of co-payment is determined by the individual practice. Consultations are typically set at 15-minute time slots; higher fees may apply if consultations are longer than 15-minutes. Please consult with your chosen practice about the information above. Frequently Asked Questions What is a Primary Health Organisation? Primary Health Organisation (PHO) is the local structures for delivery and coordinating primary health care services. PHOs bring together doctors, nurses and other health professionals (such as Maori health workers, health promoters, dietitians, pharmacists, physiotherapists, mental health workers, and midwives) in the community to serve the needs of their enrolled populations. PHOs receive a set amount of funding from the government to ensure the provision of a range of health services, including visits to the doctor. Funding is based on the people enrolled with the PHO and their characteristics (e.g. age, gender, ethnicity). Funding also pays for services that help people stay healthy and services that reach out to groups in the community who are missing out on health services or who have poor health. What are the benefits of enrolling in a PHO? Enrolling is free and voluntary. If you choose not to enrol, you can still receive health services from a chosen GP/general practice/provider of first level primary health care services. Advantages of enrolling are that your visits to the doctor will be cheaper and you will have direct access to a range of services linked to the PHO. How do I enrol? To enrol, you need to complete an enrolment form at the general practice of your choice. Parents can enrol children under 16 years of age, but children over 16 years need to sign their own form. What happens if I go to another general practice? You can go to another practice or change to a new practice at any time. If you are enrolled in a PHO through one general practice and visit another practice as a casual patient, you will pay a higher fee for that visit. So, if you have more than one general practice you should consider enrolling with the practice you visit most often. What happens if the general practice changes to a new PHO? If the general practice changes to a new PHO the practice will make this information available to you. What happens if I am enrolled in a practice but don’t see them very often? If you have not received services from your general practice in a 3 year period it is likely that the practice will contact you and ask if you wish to remain with the practice. If you are not able to be contacted or do not respond, your name will be taken off the practice and PHO enrolment registers. You can re-enrol with the same general practice or another general practice and the affiliated PHO at a later time. How do I know if I’m eligible for publicly funded health and disability services? Schools work also includes the 'Seasons' 8 week programme for young people experiencing loss/grief. A mindful living four week programme is also offered - this is aimed at older people mainly living in residential settings. The health promoter is involved in numerous community networks/activities across East Auckland. Older People's Health East Health's Intergrated Care Coordination service provides a coordinated approach to the health care of older people. Intergrated Care Coordination supports older people to live independently in the community. It can provide you and your family/whaanau with information about health and community services for older people and link you to the appropriate services. It offers health information to community groups about ageing positively. Your doctor and practice nurses work closely with the Integrated Care Coordination team and can refer you to our service. Sign up for a FREE programme: Physiotherapist Lead Choose from an individual home-based programme, pool exercise classes, or both Advanced Osteoarthritis Physiotherapy individual visits (Joint Replacement Alternative Programme) Key information The aim is to: Provide you with information on Osteoarthritis so you can manage your condition better Improve your general fitness Improve your knee and hip movement Improve your knee and hip strength Improve your general wellbeing Decrease your joint pain The Programme is free of charge. A physiotherapist will visit you at your home 5 times over a period of 3 months: Your knee and/or hip movement & strength will be checked You will be provided with information on your condition and how to best manage it You will be provided with a home exercise programme Are you eligible? You have Osteoarthritis of the hip and/or knee, have been for an Orthopaedic assessment and have been told you are not eligible or severe enough to have joint replacement surgery You have chosen not to have or unsuitable for joint replacement surgery What happens after you sign up? Upon receipt of your application, your GP will be notified and asked to verify the information you have provided to ensure you are eligible for the class. You will be contacted by a physiotherapist to offer you an initial assessment in your home Your GP will be provided with your final assessment details (after 5 sessions). Sign-up now! Osteoarthritis Pool Class Key information The aim is to: Improve your general fitness Improve your knee and hip movement Improve your general wellbeing Decrease your joint pain The Programme is free of charge. Classes take place on Monday and Thursday afternoons at the Pakuranga Park Village Retirement Village pools. The class runs for 40 mins. You are eligible for 8 weeks of classes (16 sessions). You don’t need to be able to swim as you stand up and can hold onto the edge of the pool during the entire session. Class content: a warm-up, cardiovascular conditioning, strengthening exercises, and stretches. Expectations: You are expected to attend all sessions, unless pre-arranged with the physiotherapist prior to starting the programme or if you are unwell. If you are unable to attend 2 or more sessions due to reasons other than medical (for eg going on holiday) and no arrangement has been made, your place will be offered to the next person on the waiting list. Are you eligible? You have knee or hip (or both) Osteo Arthritis You have no medical conditions preventing you from doing water-based exercises What happens after you sign up? Planned Proactive Care Planned Proactive Care (PPC) is a flexible programme for people with long term conditions. This programme aims to improve health for people diagnosed with a chronic medical condition such as: Diabetes Cardiovascular disease COPD (emphysema) Heart failure Depression Pain and others Your doctor or nurse will work with you to help manage your health. Part of the programme involves setting health goals. These health goals may be around lifestyle, medications or activity, for example. Your health goals may include 'I want to walk for 15 minutes 3x week' or 'I want to better understand how my medicines work'. Your doctor and/or nurse will help you to set goals that are realistic, measurable and achievable. Your active participation in managing your health is a key aspect. This is self management - your doctor or nurse may speak to you about the self management programmes East Health provide to patients with a range of conditions, and their families. These are informative, practical sessions to help people manage their own health more effectively. Care Plans An electronic Care Plan is part of the programme where your health goals will be recorded. To learn more, go to sharedcareplan.co.nz .
